Salvin Dental Return Policy
All returns must be accompanied by a Return Merchandise Authorization Number (an “RMA”) issued by Salvin Dental Specialties.

Returned Merchandise to Salvin, without an authorized RMA number, will NOT be eligible for credit and will NOT be returned to the sender.
All Merchandise must be returned within 45 days of buyer receipt. Items must be returned in new, resalable condition in the original packaging; only unopened, unused products will be accepted for return.

- Any Merchandise that expires within twelve months
- Refrigerated or Hazardous Merchandise
- Schedule II (CII) Merchandise (i.e. Fentanyl, Morphine, etc.)
- Special Order Merchandise
- Discontinued by Manufacturer Merchandise
- Open or Partial containers from original purchase size.
- Misbranded Merchandise that has been written on or misbranded in any way
- Any Merchandise not purchased from Salvin.

- Wrong Merchandise Received – Merchandise shipped in error Salvin must be reported within 30 business days after receipt to receive credit. Please repackage appropriately. Salvin will pay for return shipping charges.
- Missing Merchandise – Shortages must be reported within 30 business days after receipt to receive credit.
- Damaged Merchandise – Damaged Merchandise must be reported within 30 business days after receipt to receive credit. All Damaged Merchandise returned to Salvin will be subject to inspection and approval prior to issuing a credit. Salvin will pay for return shipping charges.
- Merchandise Ordered in Error – Salvin must be notified within 30 business days after receipt to receive credit. Merchandise must be returned to Salvin at the buyer’s expense. Restocking fees apply.
- Overstock – Merchandise must be reported within 30 business days after receipt to receive credit. Please repackage appropriately. Merchandise must be returned to Salvin at the buyer’s expense. Restocking fees apply.

All returned merchandise must be accompanied by a signature acknowledging the product was stored and handled in the appropriate manner and with care, along with Return Form 1030-1F-A Salvin Return Information Form filled out completely. The return will NOT be eligible for credit and will NOT be returned to the sender if these documents do not accompany the shipment.
Product(s) must be returned via FedEx or UPS to the address below. The RMA number needs to be clearly printed on the outside of the package in order to be accepted for return.

Return shipping charges are the responsibility of the customer unless otherwise noted above.
Care should be taken when packing ALL returns to ensure that they will be received in the same condition as the original shipment. Damaged merchandise will not be accepted for return or exchange.
